The WALK HOP program: a French innovative multicentric home-based cardiac rehabilitation initiative

Background Cardiac rehabilitation (CR) has demonstrated scientific benefits in terms morbidity and mortality. In France, 2019 national data indicates that only 22,3% coronary patients benefit from CR. The development alternative programs to center-based cardiac (CBCR) must be considered as a public health priority requires reorganization ambulatory care pathways. Purpose objective the "Walk Hop" program is demonstrate home-based tele-rehabilitation (HBCT) complementary safe approach CBCR, able increase proportion benefiting program. Methods This feasibility study started begin 2022. 3,200 will recruited 8 participating centers (CRC). Total duration experiment 42 months. financially supported by French authorities. HBCT solution developed for proposed CRC cardiologist. A multidisciplinary platform consisting secure portal an application allows bidirectional communication. Patients are equiped with cycle ergometer validated heart rate monitor. presents aerobic muscle strenghtening activities therapeutic education collects sessions carried out, adherence program, symptoms quality life. Results By october 2022, 157 active have been included large majority men (86%). Mean age 53,9 ± 9,0. Hypercholesterolemia noted 36,9% patients, smoking 31%, arterial hypertension 30,6% diabetes 11,1%. Obesity/overweight affects 60% population. time angioplasty initiation 81 90 days. number home 21 total 3342 realized. No cardiovascular event or rehospitalization registered since beginnng. prescribed 80 20% mean power gain at end 14,6 9,9 %. 94% expressed satisfaction high Conclusion(s) Walk Hop evaluating acceptability remote monitoring digital assistance smartphone. preliminary encouraging view final expected impact: greater cost-effective access removing obstacles like distance resumption work. It also provide improvement long-term maintenance benefits.
